Proof of Payment Collection Form

Procedure for submitting proof of payment:

1. Upload the image/photo file of your proof of transfer/transaction to your Google Drive (same as when you filled out the registration form).

2. The uploaded file should be in PNG or JPG format.

3. Copy the file link from your Google Drive (don't forget to give ‘viewer’ access to anyone who has the link).

4. Go to the Proof of Payment Collection web page.

5. Fill in the data on this form completely.

6. Paste the Google Drive link containing your proof of transfer.

7. Press the "Submit Proof of Payment" button.
